Enrollment Procedure (Preschool)
Step 1
Complete the Application for Admission for each child you wish to enroll in Madonna del Sasso using the following link https://md-ca.client.renweb.com/oa/?memberid=13405.
The following documents need to be submitted along with the Application for Admission:
*Birth Certificate
*Immunization Records
*Baptismal Certificate (if applicable)
*Copy of the most recent report card (grades 1-8)
*Copy of IEP (if applicable)
Step 2
Download and complete the mandatory State Preschool packet.
Step 3
Return the Mandatory State Preschool Packet to our school office or via email to information@mdsschoool.com. Our office is open Monday – Friday, 8:00am – 4:00pm
Step 4
Admission Acceptance: Upon acceptance to Madonna del Sasso School, you will receive an acceptance letter two weeks after testing. A Registration link will be provided to you for completion via email.
